TAKE CARE : Preventing Iatrogenic Dependence Related to Hospitalization in Elderly Patients with Neurocognitive Disorders
The number of dependent elderly people is expected to double in France between 2010 and 2060, from 1.1 to 2.3 million. Hospitalization precipitates dependency due to care and an environment that is not adapted to the needs of the elderly, particularly when they have AAD. Iatrogenic hospitalization-related dependence (IDLH) affects 10 to 30% of elderly patients, i.e. at least 150,000 elderly patients per year in France, but could be avoided 8 times out of 10, by screening, correcting and preventing 6 key factors: immobilization, confusion, malnutrition, falls, urinary incontinence and adverse drug reactions.
The Take Care research project led by Jacques Boddaert aims to prevent iatrogenic hospitalization-related dependence (IDLH) in elderly people with AAD admitted for an acute pathology, by targeting the 6 key risk factors related to the patient and by improving hospital care.
The program is based on personalized medicine and the identification of individual vulnerabilities and the analysis of the inadequacy of structures and care pathways.
The objective is to create a paradigm for the care of the elderly to prevent IDLH during acute hospitalization that can be disseminated to all healthcare structures in France.
